Just as Ellen thought she needed to find a way out of the wedding without ruining the whole clan, she got the bombshell of the rumors in Outlander: Blood of My Blood. Now the Grants have a demand that she can’t possibly face.

The Grants have demanded a purity test to make sure that her virtue is still “intact.” Of course, this means that they want to make sure she is still a virgin, and we know that she isn’t. She probably never thought that anyone would seriously question her virtue, but here she is with a sudden decision to make.
She can’t refuse, otherwise it will bring up questions about her honor. That would cause more damage to her clan than Dougal being involved with the Jacobites would. It would do more damage than the MacKenzies being close to destitute would. So, she needs to figure out a way out of this.
Brian and Ellen need to run away with each other in Outlander: Blood of My Blood
There is only one real way to get Ellen out of the degrading purity test. And it is degrading—and ridiculously, it’s something that can still be demanded of today in some parts of the world. Plus, who’s to say that it would even be in the MacKenzie’s favor even if she was still a virgin?
Ellen only has one option right now, and that’s to run away with Brian. However, she needs to find a way to get news to him that she needs to run away with him. The best option would be to get word to Glenna, since she knows of the handfasting, who can get word to Murtagh. She has no idea that Murtagh is angry with Brian right now, but this ask could end up mending fences.
What she doesn’t realize is that Brian knows that the doubts over Ellen have been cast. She also doesn’t know that Simon Fraser knows about her and Brian, and that he’s not angry with the situation at all. In fact, he’s happy with the entire thing, and this could open the door for Brian and Ellen to run away.
Simon Fraser could tell Brian to get Ellen
Brian isn’t exactly going to do anything against his father’s wishes at the moment. In Outlander: Blood of My Blood season 1, episode 7, Brian stood up to his dad, only for his dad to threaten his mom. Yes, Simon knows Brian’s weak spot, and he will use it against him.
Knowing the punishment that Davina could face, I doubt that Brian would just choose to run away with Ellen. However, he could get Simon’s permission, or even suggestion, to do it. Simon could even offer a place to hide out.
Outlander has never gone into the details of the time Ellen and Brian were hidden away until Ellen’s pregnancy showed. It would have made sense for someone to know about them to help them remain hidden, and Simon is currently the perfect person for that. With Ellen in the Fraser family, it cements the two clans together in the future, which could help Simon Fraser get what he wants.
